Accident Investigation
Introduction
In the event of an accident, companies and organizations in many industries rely on external investigations.
These external investigations or reviews are used to determine the cause of the accident and formulate

When to Conduct an Investigation
An external investigation is recommended if a fatality or disabling injury occurs. There may be other

member or where media attention or legal action is probable. Top management of an organization should
determine if an external investigation is warranted. To facilitate the collection of relevant information
the investigation should begin as soon as possible after the accident.
Conducting an Investigation
If an external investigation is warranted the top management should select an investigator or investigation
team. If a legal action is likely, and an organization so desires, it may be possible to protect the results
of the investigation from being revealed during the legal discovery process. Your organization should review
this matter with your legal counsel.
